In 2018, Penn State College of Medicine was awarded a REACH grant from the Centers for Disease Control and Prevention (CDC) to improve key chronic diseases prevention programs serving Hispanic and Latino populations in Lebanon and Berks counties. We are excited to collaborate with community partners in these counties to implement locally tailored practice- and evidence-based strategies related to our three priority areas: healthy food access, physical activity, and community-clinical linkages
